Freeze YOUR MEMBERSHIP
Please keep in mind we charge a 10% membership fee per month you are frozen.
We do not allow partial month or indefinite freezes, and you cannot cancel your membership from a freeze.
Your membership will automatically reactivate after the months listed on this form.
Cancel your membership
We’re bummed to hear you want to cancel your membership at Pharos Athletic Club and we’d would love to get feedback on what we could have done better to serve you.

Note our cancellation policies:
- If you are still within your initial 12, 6, or 3 month commitment, our cancellation fee policy will apply: If member chooses to cancel membership before the initial specified commitment is completed, a cancellation fee of two (2) months fees will incur and be charged on the termination date.
- After the completion of the initial commitment, no cancellation fee will incur, BUT a thirty (30) days written notice (on or before the 1st of the month) is required. Any scheduled membership payments within the 30 day notice period will be processed and will not be prorated.
